How To Obtain An Oklahoma Birth Or Death Certificate

Certified copies of Oklahoma birth and death records are available at the Oklahoma State Department of Health Vital Records Service. Birth certificates and death certificates were first recorded by the Division of Vital Statistics in the latter part of October 1908.

Oklahoma birth and death records prior to 1908 might be found at one of the following locations;
Oklahoma Historical Society, Dawes Commission Report or the United States Census Bureau.

How To Request A Birth Or Death Certificate.

Birth records are confidential and qualified applicants for birth records include the registrant, next-of-kin, or authorized agent, who must provide a statement that he or she is acting in the interest of the registrant. Certified copies of death certificates are available to all applicants upon completion of an application and payment of the fee charged for a search of the records. The records are not open for public inspection. County health departments do not maintain these records.

Certified Copies of birth and death records may be obtained by either:

1. Coming to the Vital Records Service, Room 117, Oklahoma State Department Of Health, 1000 Northeast 10th, Oklahoma City, OK, or Vital Records Service, Tulsa Office, 108 N. Greenwood Street, Tulsa, OK Monday - Friday, between 8:30 a.m. and 4:00 p.m., OR

2. Writing to:

Information required to process a request for birth or death certificates includes:

The name on the birth or death record being requested.
The date and place of birth or death.
Father's full name, (last, first, middle).
Mothers full name, including her maiden name.
Relationship to the person whose certificate is being requested and purpose for needing the copy.
Your daytime telephone number with area code.
Your handwritten signature and complete return mailing address.
A photocopy of your picture ID, such as your driver's license.
In addition to your written request, you must include the necessary fees. Please make your check or money order payable to the Vital Records Service.

Fees

The fee for a birth certificate is $5 and death certificates are $10 per certified copy. This fee is non-refundable or transferable if a record is not found.

Expedited Service

Requests using the U.S. Postal Service's overnight, next day delivery, will allow processing within 24 to 48 hours. Birth or Death certificates cannot be faxed.

Marriage and Divorce Certificates

All marriage and divorce records are on file with the court clerk of each county in which the marriage or divorce was granted.
